HOW TO BOOK

1. Select whether you are a New Patient or a Follow-up Patient. 
2. Click 'Add to Cart'.
3. Proceed to check-out. 
4. Fill-out your information.
5. Choose preferred mode of payment. Cash-on-Delivery (COD) is NOT applicable.
6. For NEW PATIENTS: Once paid, an email will be sent containing a downloadable Email Consultation Form. Kindly download it, read the instructions, and fill out the form at your most convenient time. 
7. For FOLLOW-UP PATIENTS: Please allow us to get back to you within 72 hours (excluding weekends & regular holidays) to review your chart. Please fill out the form sent to you. The Online Consultations Team will get in touch with you after sending us the completed form.

REMINDERS

1. Further instructions will be given through the email. Kindly send back the Email Consultation Forms within 7 days after receiving it. 
2. Cut-off for evaluation: Mondays to Saturdays, 12NN. Correct email consultation forms and photos received before 12NN will be queued for evaluation the same day (except Sundays & regular holidays). Beyond this Consultation forms will be queued the next business day. 
3. Treatment plan &  prescription will be sent by Dr. Mike via email within 72 hours from the date of queuing.
